Member Coinsurance – Your share of the costs of a covered service, after the deductible is met, calculated as a percent of the allowed amount for service.

Member Out-of-Pocket Maximum – The most you could pay during a calendar year for your share of the cost of the covered services. Plan deductible contributes towards the family out-of-pocket max. Once the family out-of- pocket max has been met, the plan will pay each eligible family member’s covered expenses at 100%.

Exclusive Provider Organization (EPO) plans contract with a network of doctors, hospitals and other healthcare facilities and professionals. These providers, called network providers, agree to special pricing arrangements. This plan generally does not provide benefits for any services you receive from a non-network provider.

RxBenefits manages your pharmacy benefits coverage, which is provided through OptumRx. RxBenefits provides service and support for King Arthur and oversight of OptumRx to maximize your benefit.

RxBenefits partners with the country’s largest Pharmacy Benefit Managers (PBMs) to bring you greater discounts, enhanced access, and improved Member Services. You will have access to the RxBenefits Member Services Team, available Monday through Friday, 8 a.m. to 9 p.m. Eastern Time. Their knowledgeable representatives can assist you with all of your pharmacy-related questions.

RxBenefits Member Services reps have access to the OptumRx systems. If RxBenefits needs to contact OptumRx to resolve an issue, they will stay on the line, explain the issue, and continue to monitor your problem until it is resolved.

FAQs

Are my drugs covered?
Your medications may not be in the same tier level under OptumRx as they were under your prior plan, so please review the preferred drug list since it may provide lower cost alternatives for your medications. Also, discussing generics with your physician could save you money.

Health Savings Account Information

What is a Health Savings Account?

A Health Savings Account or HSA is a tax-advantaged savings account owned by an individual that can be used to pay for qualified medical expenses for the owner and their dependents. An HSA, which must be paired with an HSA-qualified health plan (the Bronze Plan), allows employees to make pre-tax contributions into a federally-insured account. HSA balances earn tax-free interest, roll over from year to year and can be invested to accelerate growth.

All the money in an HSA is owned by the employee even if they leave their job, lose their qualifying coverage or retire. The money in an HSA never expires. All remaining HSA funds roll over each year.

Employer Contribution

KAB will match employee contributions to an HSA up to a maximum of $20 per pay period (or $520 annually). Employees can indicate how much they want to contribute to an HSA during the ADP online enrollment process.

HSA Eligibility

To be eligible for an HSA, an employee must:

  1. Be enrolled in a qualified High Deductible Health Plan (HDHP) (ie. HSA (Bronze) Plan)
  2. Not be claimed as a dependent on another’s tax return
  3. Not be enrolled in Medicare
  4. Not be insured under any other medical plan that is not a HDHP
  5. Have not received VA benefits (Medical or Rx) during that past 3 months

HSA Qualified Expenses Include but are not limited to:

Your Flexible Spending Account Benefits

King Arthur Baking Company offers three different types of flexible spending accounts. Employees may contribute pre-tax dollars into these accounts to help offset eligible medical expenses or dependent care expenses. The plans are administered by EBPA Benefits.

General Purpose Flexible Spending Account (FSA)

Funds from a general purpose FSA can be used for qualified expenses including deductibles, co-payments and coinsurance. With an FSA, the entire elected amount is available on the first day of the health plan year.

  • Maximum contribution: $5,000 (if single or married & filing jointly) or $2,500 (if married & filing separately)

Employee-owners are offered two comprehensive dental plan options through Northeast Delta Dental. A Basic Plan with a $750 calendar year plan maximum per person and an Enhanced Plan with a $2000 calendar year plan maximum per person.

KAB employee-owners may see any provider to receive your dental care. However, if you select a dental provider who is part of the Delta Dental PPO or Delta Dental Premier network, you will receive dental services at a negotiated rate that will typically be less than outside the network and may also receive higher reimbursement rates.

Basic Employee Life Insurance and Accidental Death & Dismemberment (AD&D)

King Arthur Baking provides Basic Life and AD&D insurance through Unum. The benefit is provided at no cost to eligible employee-owners:

  • Life insurance equal to two times your annual salary to a maximum of $200,000
  • AD&D insurance equal to two times your annual salary to a maximum of $200,000

Employer Provided Life Insurance and Imputed income
IRS rules allow employers to provide up to $50,000 of life insurance to employee tax-free. However, the IRS requires that employer subsidized life insurance coverage over $50,000 be taxed. The taxable cost of the coverage over $50,000 is known as “imputed income” equals the estimated cost that the IRS assumes you would pay to purchase an individual policy in the same amount as your employer-subsidized coverage over $50,000. If you have imputed income on your life insurance coverage, it will be added to the taxable earnings reported on your paycheck.

Benefits payable under employee basic life insurance are reduced to 65% at age 65, 42% at age 70, and 27% at age 75.

Employee Optional Life Insurance

You may purchase optional life insurance in $10,000 increments to a maximum of five times your annual salary or $300,000, whichever is less.

If you enroll in this benefit when you first become eligible, then you may elect up to $150,000 without providing evidence of insurability. If you elect an amount above the guarantee issue amount or if you decide to enroll or increase your coverage after your initial eligibility date, you will be required to provide evidence of insurability.

Benefits payable under employee optional life insurance are reduced to 65% at age 65, 42% at age 70, and 27% at age 75.

Spouse Optional Life Insurance

You may purchase life insurance for your spouse in $5,000 increments to a maximum of $100,000, if you have elected voluntary life coverage on yourself first. If you enroll in this benefit when you first become eligible you may elect up to $30,000 without providing evidence of insurability for your spouse. If you elect an amount above the guaranteed issue amount or if you decide to enroll or increase spousal coverage after your initial eligibility date, your spouse will be required to provide evidence of insurability.

To elect this coverage, you must elect optional coverage for yourself. The maximum spousal life amount may not exceed 100% of the optional employee coverage.

The benefit payable under the spouse coverage is reduced to 65% at age 65, 42% at age 70, and 27% at age 75.

Child Optional Life Insurance

You may purchase coverage for your eligible dependent children. The benefit is equal to $250 for children aged 14 days to 6 months, and $10,000 for children aged 6 months to age 19 or age 25 if a full time student at an accredited educational institution. To elect coverage for your children, you must elect optional coverage for yourself.

Benefit Details

Short-Term Disability Insurance

King Arthur Baking provides Short-Term Disability insurance through Unum. The benefit is provided at no cost to eligible employee-owners:

  • Benefit is 60% of your basic weekly earnings for up to 13 weeks
  • Minimum benefit is $25
  • Benefits begin on the 1st day for disability due to an accidental injury
  • Benefits begin on the 8th consecutive day of illness for disability due to sickness or pregnancy
  • Payments of this benefit will be paid on a biweekly basis through King Arthur

Long-Term Disability Insurance

King Arthur Baking provides Long-Term Disability insurance through Unum. The benefit is provided at no cost to eligible employee-owners:

  • Benefit is 60% of pre-disability earnings to a maximum of $6,000
  • Employees must be disabled for 90 days before benefits are paid
  • Payments of this benefit will be paid monthly, directly from Unum
  • Includes travel assistance (assistance for missing passports, lost credit cards, and stolen cash)

Your Accident Benefits

Eligibility

King Arthur Baking offers employees the option of enrolling in a Voluntary Accident plan.

Voluntary Accident Insurance provides a range of fixed, lump-sum benefits for injuries resulting from a covered accident. These benefits are paid directly to the insured and may be used for any reason, from deductibles and prescriptions to transportation and childcare.

Your Critical Illness Benefits

Eligibility

Benefit Details

King Arthur Baking offers employees the option of enrolling in a Voluntary Critical Illness plan.

Voluntary Critical Illness Insurance provides a fixed, lump-sum benefit upon diagnosis of a critical  illness, which can cause heart attack, stroke, paralysis and more. These benefits are paid directly  to the insured and may be used for any reason, from deductibles and prescriptions to  transportation and childcare.

Employees can choose to cover themselves in increments of $10,000, $20,000, or $30,000. Employees can choose to cover their spouse in the same increments, not to exceed employee coverage. Dependent  child(ren) are automatically covered at 50% of the employee coverage amount at no additional cost.

Employee per pay period contribution amounts are based on the employee’s age for both employee and spouse coverage.

Be Well Benefit

Included with Critical Illness, Unum’s Be Well benefit is a yearly benefit payable directly to you for one covered health screening per year, per covered individual.

Be Well is a financial incentive to take a wellness test, which can detect common issues and warning signs to give you the head start you need to improve your health.

The Be Well benefit provides $50, $75 or $100 per enrolled depending on the Critical Illness benefit elected.

The KAB Learning Platform (through ADP) is designed for employees to access skill building courses as well as enroll in Brainfood sessions, access internal training for new hires, critical compliance, Safety and Quality information.  Located on ADP, we will be developing this platform over the next year and invite employees to explore learning opportunities.  Below is some general information about using the platform.

 

What can I find on the Employee Learning Platform?

There are three main types of learning content that can be found in the platform:

  1. Over 70,000 on-demand skill building courses available for self-enrollment
  2. Brainfood library where you can review and enroll in sessions of our Brainfood courses
  3. Required KAB training such as safety training, harassment training, etc. Employees will be notified directly when these required courses are assigned

 

 Can I complete courses on paid time?

In some cases, yes. For courses which are required (either you are enrolled by your manager or HR/safety), you should complete them during paid time. You should work directly with your manager to determine when best to do this.

If an employee finds a course which they believe will contribute to their development in their current role, they should discuss directly with their manager. If the manager agrees, they may enroll you in the course to allow you to take it during paid time.

Additional courses employees may choose to enroll in that do not meet the above parameters should be taken outside of paid time.

 

How do I access the Employee Learning Platform?

The Employee Learning Platform can be found in ADP.

1. First log into ADP using your standard log in credentials

2. In the Quick Links widget on your homepage, click “My Learning”

  • Alternatively, from anywhere in ADP you can also navigate to this page by dropping down “Myself” in the upper ribbon, and then clicking “My Learning” within the “Talent” section

3. Once in the Learning Platform, at the top you will see listed all courses which you are currently enrolled in. From here you can access your already enrolled courses.

  • Scrolling down, you will next see our curated courses, such as Brainfood. Clicking here will allow you to view the full Brainfood catalogue.
  • Below curated courses, you will find recommended skill development courses available for self-enrollment
  • You can always search the course library using the search bar and use filters to view based on a variety of topics.
